Swelling of the lymph nodes
Swollen lymph glands are caused by a viral or bacterial infection in the lymph nodes. Nodules are a very important part of the immune system that fights infections. This disease can also be manifested by lower back pain.
Treatment: if a bacterial infection is the cause of enlarged lymph nodes, then the solution is antibiotic treatment. Causes of Back Pain
In Russian neurology, back pain is often attributed to protrusion or hernia. But the whole world thinks differently, and I try to convey to you modern world views in a simple language.

The same thing happens with our spine. In most cases, protrusion and hernia are natural age-related changes. If in your life there are a lot of provoking factors, for example, carrying heavy loads, then the probability of getting a large hernia increases.
Numerous studies have found that protrusion of the disc occurs in healthy people: at 20 years old – up to 40% of patients (I recall, these are patients without back pain), at 70 years old – up to 90%. In addition, the severity of changes in MRI is not related to the degree of pain: with a large hernia, there may not be pain at all, and with small protrusions the patient may have severe pain (because the reason is not protrusion, but in another).
Does a hernia never cause pain?
Of course it does! About 1-4% of all cases of back pain.
How to recognize it?
A hernia leads not only to pain, but also to other disorders: a violation of sensitivity, a change in reflexes, and sometimes a decrease in muscle strength. A competent doctor determines all this during a neurological examination. If the muscle strength is sufficient, the reflexes when tapping with a hammer are symmetrical, not weakened, there are no sensory disturbances, then it is extremely unlikely that the back pain is caused by a hernia.
In addition, squeezing the spinal root at a certain level (if you read the MRI results, you will see that protrusions and hernias are described at a level, for example C3-C5 or L5-S1), the hernia does not just decrease sensitivity anywhere, but in certain segments, and strictly defined changes in reflexes. But why does the back hurt if the hernia / protrusion is not to blame?
All causes of back pain can be divided into 3 groups: non-specific, specific and radicular.

How to recognize?
During a neurological examination, there are no alarming symptoms and signs of damage to the nerve roots, the pain intensifies with movement, decreases at rest, most often associated with an incorrect motor stereotype.
Acute pain occurs after an unsuccessful turn, tilt, forced posture, for example, sleeping in an uncomfortable position – and the next morning it is impossible to turn the neck. Chronic pain is often due to the fact that a person puts too much strain on some muscle groups and does not burden others at all.
Long work in a sitting position, carrying a child on one side are incorrect motor stereotypes. You can drink pills endlessly, but until you change your physical activity, it is likely that the pain will return. Of course, activity should be wise.
neither x-rays nor MRI are needed, only a doctor’s examination. Why? They either will not find anything, or they will find protrusions / hernias that are not relevant.
Specific – up to 10%: spinal fracture, tuberculosis, osteomyelitis, abscess, spinal stenosis, neoplasm, ankylosing spondylitis and others.
How to recognize?
In addition to back pain, there may be other symptoms (fever, weight loss) and the pain itself has unusual characteristics, for example, does not depend on movement or intensifies at rest, at night.
A blood test, x-ray, MRI, PET-CT – it all depends on the alleged cause.
Radicular – up to 4% (according to some sources up to 10%), caused by compression of the nerve root due to a hernia.

When should back pain alert me?
There is a system of “red flags” – symptoms that should cause diagnostic alertness in a doctor:
- the occurrence of pain with new, unusual characteristics at the age of less than 20 or more than 55 years;
- pain independent of movement, pain at rest;
- increased pain at night;
- the nature of pain increasing over time;
- lack of pain relief or its intensification after being in a supine position;
- localization of pain in the thoracic spine or in the chest;
- prolonged malaise, fever, unexplained weight loss.
Anamnesis (history of life and disease)
- recent back injury;
- the presence in the history of cancer (malignant) diseases;
- persistent infection;
- prolonged use of corticosteroids (including in the form of inhalation);
- the use of anticoagulants;
- intravenous drug use;
- immunodeficiency (including HIV infection), taking immunosuppressive drugs.
- the presence of focal neurological symptoms in the patient (limb weakness, numbness, impaired urination and defecation);
- pain on palpation and percussion of the spine, increased pain when straining;
- severe spinal deformity;
- pulsating abdominal mass.

The presence of these symptoms (especially the totality) in a patient with back pain should make the doctor think better about the cause of the pain and rule out serious pathology.
Is it necessary to do an MRI for back pain?
With MRI for back pain, there are 2 extremes: do it all in a row “just in case” and do not do research when it is really needed. We deal with you.
If pain in the lower back occurred in a patient younger than 50 years old, it does not radiate to the legs, is not accompanied by fever and neurological symptoms (muscle weakness, impaired reflexes and sensitivity, impaired urination and defecation), there were no tumors in the past, then with a probability of 99 % pain is non-specific and is associated with the involvement of muscles, ligaments, tendons, small joints.
To bring this probability closer to 100%, we evaluate the patient in dynamics and see if the pain decreases after treatment or not. If an MRI of the spine is made for such a patient, we will not receive any important information.
Even if hernias or protrusions are found, this will not affect anything.
Firstly, in a patient without neurological symptoms, hernias and protrusions are an accidental finding and are not associated with back pain. This idea is so difficult to take root in the minds of patients and colleagues, but nevertheless it is.
Secondly, findings on MRI in the form of dumb, silent hernias and protrusions will not affect therapeutic tactics in any way.
But how not to miss another serious pathology? There are red flags for this that we have already discussed. The doctor analyzes all these alarming symptoms (in aggregate, and not separately), after which he makes a decision on the need for MRI.
